The Ghanaian champions arrived in the Democratic Republic of Congo needing to overturn a slim 1-0 deficit from the first leg in Tarkwa, but Mazembe’s pedigree on home soil proved decisive.
Patrick Mwanungulu opened the scoring within the first 15 minutes, and Kazema Baso quickly added a second to put the hosts firmly in control.
Mwanungulu struck again after the break to seal his brace and stretch the aggregate margin beyond Medeama’s reach.
Dominic Amponsah netted a late consolation for the Mauve and Yellow, but it was not enough to alter the outcome.
The defeat marks an early exit for Medeama, who now shift focus to defending their Ghana Premier League crown. Meanwhile, Mazembe, the five-time African champions, advanced to the next preliminary stage, where they will meet Zambian side Red Arrows.
